What You’ll Do The Community Health Specialist plays a key leadership role in the Constellation Community Care Hub (CCCH), leading efforts to integrate community-based services with clinical care. This position is responsible for managing care hub operations, supporting the sustainability of the Community Health Worker (CHW) workforce, and cultivating and training partners across the care ecosystem to strengthen whole-person care and address social drivers of cardiovascular disease. Oversee day-to-day operations of the Constellation Community Care Hub, including care coordination workflows, integration and tracking of bi-directional referrals, quality improvement initiatives, and collaboration with partners across South Carolina. Supervise and/or mentor CHWs, providing onboarding, training, coaching, and performance support to ensure consistent, high-quality service delivery across clinical and community-based settings. Build and maintain strong partnerships with healthcare providers and Community Based Organizations (CBOs); conduct community outreach and lead efforts to enhance health literacy and equitable access to care. Provide training and technical assistance to partners on CHW workflows, documentation, motivational interviewing, resource navigation, and Medicare billing models (e.g., CHI, PIN, SDOH). Establish and maintain effective working relationships with internal teams, healthcare providers, and community partners. Manage multiple tasks simultaneously, work well in team environments, meet deadlines, and operate independently. Regional travel is REQUIRED in South Carolina in Anderson, Berkeley, Darlington, Florence, Lancaster, Lee, Richland, Spartanburg, Sumter, Williamsburg, and York counties and other identified counties/regions in the Innovative Award service area to meet patients, engage with community stakeholders, attend conferences and relationship building; therefore, candidates must reside in SC. Who You Are The specialist must be aCertified South Carolina CHW (CHW II or CHW Supervisor) with demonstrated experience in supervision, community engagement, and care coordination. The ideal candidate will be a systems thinker and experienced CHW leader, capable of guiding a growing care hub model, maintaining partner relationships, and advancing equity-focused strategies across the region. Our requirement for this role: Active South Carolina Certified Community Health Worker II or CHW Supervisor certification is required. Candidates with current South Carolina licensure as a Licensed Bachelor Social Worker (LBSW) or Licensed Master Social Worker (LMSW) may be considered if they have relevant community-based experience and meet the leadership criteria. A background in public health, social work, healthcare, or health education is strongly preferred. Minimum of 4 years of CHW, social work or community-based health experience, with at least 2 years in a supervisory or lead role. A bachelor’s degree in public health, social work, community health, health education, or a related field, or experience supporting programs focused on cardiovascular health, chronic disease prevention, or health equity is preferred. Demonstrated experience in care coordination, CHW program implementation, and cross-sector collaboration; knowledge of Medicare/Medicaid billing processes (e.g., CHI, PIN, or SDOH billing) is a plus. Strong understanding of healthcare systems, social determinants of health, and integrated care models is essential. Experience working within value-based care or managed care environments is preferred. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and comfort with digital documentation systems is required. Familiarity with Electronic Health Records (EHRs), care management platforms, or referral systems is a plus. Excellent verbal and written communication skills, including facilitation, public speaking, and professional writing, are required. Experience delivering trainings, workshops, or presentations is preferred. Self-directed with strong organizational and facilitation skills, including motivational interviewing. Ability to travel within South Carolina up to 50% to engage partners, conduct site visits, and participate in statewide meetings. Additional travel in the Southeast region to be expected.
