As a Patient Care Manager Senior, you will play a vital role in leading our clinical team and ensuring the delivery of high-quality patient care. Key Responsibilities - Preparation and facilitation of Interdisciplinary Group (IDG) meetings, including presentation of patients for review and coordination of minutes. - Management of clinical and administrative recordkeeping functions, including documentation of referrals, new physicians and facilities, durable medical equipment pickup, and Medicare patient eligibility. - Direction of the work of clinical associates, including assigning admit and evaluation visits, scheduling visits at the beginning of the benefit period, and ensuring staff are completing visits in a timely manner. About You - Graduate of an accredited school of nursing, with a bachelor's degree preferred. - Current Registered Nurse licensure in the state of residence and requested employment. - At least three years' experience in direct patient care and two years' experience in hospice or home-based care settings.
