Job Description Gentiva Hospice seeks a skilled Patient Care Manager Senior to join our team. As a member of our clinical leadership team, you will be responsible for overseeing patient care activities and ensuring the delivery of high-quality care. Responsibilities 1. Interdisciplinary Group (IDG) Meetings: Prepare for, facilitate, and preside over IDG meetings, ensuring that all relevant information is presented and discussed. 2. Clinical Recordkeeping: Manage and oversee clinical and administrative recordkeeping functions, including documentation of referrals, new physicians and facilities, durable medical equipment pickup, and Medicare patient eligibility. 3. Staff Supervision: Direct the work of clinical associates, including assigning admit and evaluation visits, scheduling visits at the beginning of the benefit period, and ensuring staff are completing visits in a timely manner. Requirements - Graduate of an accredited school of nursing, with a bachelor's degree preferred. - Current Registered Nurse licensure in the state of residence and requested employment. - At least three years' experience in direct patient care and two years' experience in hospice or home-based care settings.
